The Swiss healthcare system is facing ever greater challenges. Health costs are increasing, more people are suffering from multiple conditions at the same time, there is a shortage of healthcare professionals and health insurance premiums are steadily rising. In addition, many acute care hospitals are struggling with economic problems, and the cooperation between inpatient and outpatient care is often inefficient. Innovative solutions are therefore urgently needed.

Since 1 January 2025, the SCC has been promoting and establishing integrated healthcare through hospital-equivalent Care@home models, with the aim of providing patients with safe and effective nursing, medical, therapeutic and social care in their own homes. We link practice with academia, research, industry and politics, thus creating a strong capacity for innovation. The network currently comprises around 70 lead partners, including hospitals, home care providers, general medical practices, health insurance funds, retirement and nursing homes, outpatient health and technology providers, and other healthcare institutions.

Care@home is an integrated healthcare model that provides patients of all ages living at home or in nursing facilities with acute care as well as medical and therapeutic care. An interprofessional team of service providers treats the patients and uses digital and technological aids to achieve the following goals:

• Shortening the duration of hospital stays (early transfer)
• Avoiding hospital stays (admission avoidance)
• Improving healthcare in rural and remote regions (timely access)

As a leading house and research partner, we develop innovative intraprofessional and interprofessional as well as intersectoral care models. This includes prevention strategies and modern technologies to strengthen the independence and quality of life of patients, to avoid unnecessary hospital stays and to increase the efficiency of the healthcare system.

Mission: We are forming an active and innovative network

The SCC is committed to promoting intersectoral and interprofessional healthcare through effective and efficient measures. For this purpose, we are forming an active network in the SCC consisting of lead partners, researchers, scientists, and research and teaching experts at Bern University of Applied Sciences, as well as cantonal and national research institutions.

Objectives: What we aim to achieve

• Care@home offers treatment of a comparable or higher standard than that provided during a hospital stay, resulting in fewer complications and a high level of patient satisfaction.
• Care@home establishes intraprofessional and interprofessional cooperation as well as intersectoral collaboration, supported by seamless (digital) exchange of information between service providers. This contributes to higher job satisfaction and longer job retention among healthcare professionals.
• Effective measures ensure efficient healthcare, either by reducing costs while maintaining quality of care or by increasing quality of care while keeping costs constant. This is achieved through an integrated supply chain that connects outpatient and inpatient care providers throughout the entire patient journey.
• Care@home strengthens the canton of Bern and Switzerland as a health hub and positions the canton as a pioneer for evidence-based, functioning and integrated Care@home healthcare extending into the peripheral areas.
